What we collect
Information you give us during the eligibility check and health intake (health history, medications, goals, and contact details), information created during your care (visit notes, prescriptions, and lab results), and basic technical data such as device type and pages visited.
How we use it
To evaluate and deliver your care, to confirm that care is available in your state, to process your plan payments, and to meet legal and professional recordkeeping requirements. Health information is not sold and is not used for advertising.
Who can see it
Your licensed provider and the care team supporting your treatment; the pharmacy or laboratory that fulfills an order your provider places; technology vendors bound by business associate agreements; and public authorities only where the law requires it.
How we protect it
We use secure systems, encrypt information in transit and at rest, limit access to people involved in your care, and follow applicable privacy requirements, including HIPAA where it applies.
Your choices and rights
You may ask for a copy of your records, request corrections, limit certain uses, or ask us to delete information we are not required to keep. Email privacy@allgoodwellnessclinic.com and we will respond within the time the law requires.
Retention
Medical records are kept for the period required by the law of the state where you received care. Website analytics data is kept for a limited period and then deleted or anonymized.
